As part of Family Care Health Services, we help patients safely transition from hospital to stable living by offering temporary residential care, medical oversight, and housing support in a compassionate, structured environment.

Who qualifies for recuperative care at HealHaven?
Patients must be medically stable, unhoused or at risk of homelessness, and recently discharged from a hospital. Most are enrolled in Medi-Cal and meet eligibility for Enhanced Care Management (ECM) or CalAIM Community Supports.
How long can someone stay at HealHaven?
Stays are typically up to 90 days, based on medical need and recovery progress. Extensions may be considered in specific cases with health plan approval.
What services are included during the stay?
Patients receive housing, meals, daily medical monitoring, medication support, transportation to appointments, case management, and help navigating housing and benefits programs.
Does HealHaven provide mental health support?
Yes. We offer mental health assessments, referrals, and coordination with behavioral health providers as part of a patient’s individualized care plan.
Who can refer a patient to HealHaven?
Hospitals, discharge planners, case managers, and Medi-Cal managed care providers in Los Angeles County can submit referrals. We also accept referrals through the CalAIM program.
Is HealHaven affiliated with a hospital?
HealHaven is not a hospital but partners closely with local hospitals and health plans to support safe recovery after discharge in a non-institutional, home-like setting.
